The City of Capitola Memorial Plaque program has allowed friends and family a way to remember their loved ones in various locations around the village. Unfortunately, as a result of the recent storms some plaques and benches have been lost or damaged. Knowing that these are treasured items we are taking a careful inventory and asking the public to please return any found plaques to:

We are also requesting that family members of those with an existing plaque please fill out this form so that we can have accurate contact information.
